PURETOX MEMBERSHIP TERMS
You are paying for a monthly membership with different treatment options and benefits which you may choose to use or decline. Treatments may be purchased and used within any membership month. Treatments must be used by the member and can not be gifted or used by others. Monthly membership fees are non-refundable. You may cancel your membership at any time upon which the final payment is due if not already paid. Individuals who cancel will not be permitted to rejoin the program for a minimum period of 6 months. The PURETOX program may be changed, modified or discontinued at any time at the sole discretion of The PURE MedSpa. All treatments are provided at the discretion of the medical providers. If you are not a candidate for any of the treatments offered in the membership, you will be offered an alternative membership plan or a refund. A 24-hour notice is required for cancellation or rescheduling of appointments or you will pay a $100 rescheduling fee. You indemnify The PURE MedSpa and its staff from any liability related to your use of the PURETOX program or for any adverse reactions or outcomes resulting from treatments provided. All personal and medical information provided by members will be kept confidential in accordance with applicable privacy laws. By enrolling in the PURETOX program, you agree to abide by these terms and conditions.
