You are paying for a monthly membership with a variety of treatment options, benefits, discounts, and gifts which you may choose to use or decline. Free treatment options include Free Monthly Medical Facials upon enrollment into the PASSPORT to Pure Confidence program. Other treatment options are provided at a 10% discount with each month of membership, including but not limited to Laser Hair Removal, IPL treatments, Facials, Microdermabrasion, Laser Fat Melting, Body Sculpting, Hydrafacials, Body Toning, Injectables, Laser Treatments, and Medical Grade Skincare. All free treatments must be used within the membership month - unused treatments do not "roll over" to the next month. Treatments must be used by the member and can not be gifted or used by others. Members can use their 10% discount to purchase/gift treatments, services, and/or products to other individuals. Monthly membership fees are non-refundable. You may cancel your membership at any time upon which the final payment is due if not already paid. Individuals who cancel will not be able to rejoin the program for a period of 6 months. The PASSPORT to PURE Confidence program may be modified or discontinued at any time by PURE MedSpa. All treatments are provided at the discretion of medical providers. If you are not a candidate for any of the treatments offered in the membership, you will be offered an alternative membership plan. A 24-hour notice is required for cancellation or rescheduling of appointments. PURE MedSpa and its staff are not liable for any adverse reactions or outcomes resulting from treatments. All personal and medical information provided by members will be kept confidential in accordance with applicable privacy laws.
